Personalized Supportive Care
Sometimes, even though you’re active and engaged in life, you may need some additional support with daily living. At The Bridges at Bent Creek, each resident receives a personalized care plan based on their unique needs and preferences. Prior to moving into our community, a licensed nurse will meet with each resident and their family to complete a comprehensive assessment to determine the most appropriate level of support required. Our team will also conduct ongoing care plan meetings to identify any changes in personal care needs, support or medication management. Families take comfort in knowing that we have the ability to care for Supportive Care residents who have more challenging medical needs, which allows delaying or possibly even eliminating the need for a higher level of care. Our residents in this personalized program enjoy private apartments with supportive care as needed.

For residents who require cognitive support, we offer a personalized approach to memory care. With 24-hour security, our programming offers everything that families need to be assured their loved ones are safe. Our approach promotes individualized care based on where a person is on their journey. We address specific needs, covering everything from proper nutrition and general fitness to sensory stimulation exercises, recreational outings and time for family reminiscing.

GARDEN SECURED MEMORY CARE
The Garden is a secure, 19-suite Memory Care neighborhood for residents whose memory loss is more advanced. Our programming approach delivers purposeful activities and focuses on the satisfaction, comfort and safety of our residents. Every associate working in The Garden receives specialized training to best meet the leisure and personal care needs of each resident. In The Garden, we can care for those in various stages of memory decline and offer customized programming and care based on an individual’s needs and abilities. Residents enjoy spacious private or semi-private accommodations along with a large great room and beautiful sunroom, which are perfect for visits with family and friends.

SEASONS TRANSITIONAL PROGRAM
Designed to support residents who have early-stage memory loss but who do not need a secure environment, our Seasons Transitional program is based on the premise that activity and engagement are integral to daily living. Our goal is to encourage residents to maintain involvement in ongoing structured activities and events within a smaller group, eliminating loneliness and boredom as well as slowing the decline of cognitive skills. This program supports our residents living in Supportive Care and includes all amenities and services listed under Personalized Supportive Care.
